WHAT IS HEALTHPASS NEW YORK?
HealthPass New York is an independent, not-for-proıt, commercial health insurance exchange offering small businesses and sole proprietors in the New York metropolitan area quality, affordable, and ıexible health care coverage.

Since 1999, the HealthPass choice model has empowered employees to choose comprehensive health care coverage that ıts their unique medical needs and budget. Employees can choose different beneıt op ons from four leading carriers including four dental plans, vision, and a bundled security product.

HEALTHPASS NEW YORK FOR YOUR SMALL BUSINESS

If your company is located in the 5 boroughs of New York City, Nassau, Suffolk, Westchester, Rockland, Orange, Putnam, Dutchess, Ulster or Sullivan coun es, and if you have 2 or more full- me employees, your small business is eligible for HealthPass. Unlike other carriers, which require higher employee par cipa on in their plans, HealthPass has a 75% loose par cipa on requirement. That means any employee who already has coverage through an exis ng individual policy, spousal coverage, group sponsored plan and/or government program count posi vely towards par cipa on. This makes it easier for employers to provide health insurance. To be eligible, employees must meet certain guidelines set forth by the company. You decide the number of hours employees must work and the wai ng period which employees must sa sfy in order to be eligible. Eligible employees residing in the tri-state area (including Bucks County, PA) can enroll in a HealthPass plan. In addi on, HealthPass will cover up to 20% of eligible employees who reside outside of the coverage area.

HEALTHPASS FOR SOLE PROPRIETORS:
If your company is located in the 5 boroughs of New York City, Nassau or Suffolk counties and if you are the owner and have no other employees your company is eligible for HealthPass Sole Proprietors.

AS BOTH AN EMPLOYER AND EMPLOYEE YOU ARE ABLE TO CHOOSE:
- From 4 different Oxford medical plan options, including Rx
- Cost Sharing Plans
- Health Savings Accounts (HSAs)
- Dental Coverage
- EverGuard - Bundled Term Life, AD&D Disability Package

HEALTHPASS OFFERS IT'S MEMBERS:

Greater Access to Providers - with over 200,000 providers, there is greater flexibility to choose a plan that covers the most important doctors and hospitals in your area.

Discount & Wellness Programs - every member of HealthPass is afforded a suite of discount and wellness programs from their selected carrier. Program services may include discounts for: local gyms, diet centers, LASIK surgery and other vision benefits, acupuncture, massage therapy, dietician services, smoking cessation, hearing discounts and more.

Member Services - all employees have access to HealthPass' Member Service Team to help guide them through the questions or concerns they might have regarding their eligibility, enrollment, and benefit inquiries.

Health Advocate* - all employees enrolled in medical coverage have access to this advocacy and assistance company. Registered nurses can help guide them through questions regarding claims, healthcare bills, authorizations, finding doctors and hospitals, as well as scheduling specialist visits.

Medical Cost Advocate - all members have access to Medical Cost Advocate. These experts negotiate medical charges directly with health care providers (hospitals, physicians, labs, etc.) to reduce employees medical expenses.

Simplified Administration - 1 form handles employee enrollment, termination, waiver and COBRA/State Continuation of Coverage. 1 itemized bill is sent to employers for all lines of coverages.

COBRA /NY State Continuation Administration - HealthPass handles the administrative direct employee billing for all enrolled groups.
