TITLE: DAY SERVICES SPECIALIST

SUMMARY OF POSITION: Responsible for writing day habilitation plans, conducting team meetings to review each assigned individual's plan and summarizing individual's response to the plans by completing monthly review notes. TITLE: PRE-VOCATIONAL SPECIALIST
PROGRAM: PRE-VOCATIONAL
SUMMARY OF POSITION: Develop and implement service documents with individuals and their families; maintain a caseload based on program needs; plan, conduct, and implement pre-
vocational plans with individuals and their advocates. In addition, complete pre-vocational
paperwork and required documentation; organize, develop and provide supervision and support to individuals at job placement sites.

TITLE: PRE-VOCATIONAL TRAINER PROGRAM: PRE-VOCATIONAL
SUMMARY OF POSITION: Assign work and train individuals on the required tasks for satisfactory completion of jobs. Maintain daily records of attendance and productivity and
provide written and verbal reports on the individual's progress and performance. Assume responsibility for quality control.

TITLE: PRE-VOCATIONAL SUPERVISOR
PROGRAM: PRE-VOCATIONAL
SUMMARY OF POSITION: Develop and implement vocational instruction including
development of curriculum, ordering supplies and materials, providing training to staff. Provide supervision to the vocational coaches in the pre-vocational program. Develop, implement and oversee off-site work sites. Conduct and complete documents required for OPWDD. Develop and implement service documents with individuals and their families. These documents include valued outcomes and services provided to individuals. Plan, conduct and implement pre-
vocational plans with individuals, and their advocates. Complete pre-vocational paperwork in a timely manner.
MINIMUM REQUIREMENTS: Bachelor's degree and 2 years of supervisory experience
working in programs for adults with developmental disabilities required. A valid NYS Driver's License is required for 3 years.

TITLE: PORTER
PROGRAM: MAINTENANCE
SUMMARY OF POSITION:
Maintains general cleanliness of Agency's buildings and grounds. This includes repairing
equipment, servicing, cleaning and sanitizing restroom, removing trash daily, painting areas as necessary, and cleaning and waxing floors.

TITLE: JOB COACH
PROGRAM: SUPPORTED EMPLOYMENT
SUMMARY OF POSITION:
Provide on-site skills training for people with developmental disabilities by providing hands-on instruction/assistance in performing job duties and learning work habits and behaviors expected on their job; perform job tasks for enrollees as necessary; work as a member of an
interdisciplinary team and function as a liaison between individuals, families, work-site and agency.
MINIMUM REQUIREMENTS:
Bachelor's Degree, experience working with people with developmental disabilities and valid NYS driver's license required. Own car preferred.

RESIDENTIAL PROGRAM OPENINGS
TITLE: DIRECT SUPPORT PROFESSIONAL PROGRAM: RESIDENTIAL
SUMMARY OF POSITION:
Provides comprehensive care to adults with developmental disabilities in their homes. Works on ADL goals, assists with personal hygiene needs, performs housekeeping tasks and escorts clients on recreational activities. Various shifts available from evenings (4pm-12am), early mornings (Gam-loam or Gam-2prn), overnights (12am-8am) and weekends shifts (8am-4pm, 4pm-12am).
MINIMUM REQUIREMENTS:
High School Diploma/GED or some college, studying in the field of Human Services. A valid New York State driver's license for three (3) years required. Prior work experience in a human service field working with individuals with developmental disabilities preferred.

TITLE: ASSISTANT SUPERVISOFt/QIDP PROGRAM: RESIDENTIAL
SUMMARY OF POSITION:
The Assistant Supervisor will work in conjunction with the Residence Supervisor, in providing daily supervision in the operations of a 10 bed residence in accordance with OPWDD regulations & QCP policies. In addition, motivates & trains staff to enhance optimum services for the
individuals with developmental disabilities in their home. Assist the Residence Supervisor in writing & implementing life plans, as well as ensures that family members are included in the process. Organizes in-house & outside recreational activities.
MINIMUM REQUIREMENTS: Bachelor's Degree in human services along with one year of work experience in the field with individuals with developmental disabilities. Successful
candidate should have strong oral & written skills, strong leadership skills that include being a self-starter & the ability to work well under pressure. Requires a valid NYS Driver's License for three (3) years.

TITLE: LPN - Nights (36 hrs.)
PROGRAM: RESIDENTIAL
SUMMARY OF POSITION:
The LPN is responsible for medical services for each person living in the residences. Monitors and supervises all medical treatments, appointments, including medication, nutrition, lab
procedures, etc. Ensures that a comprehensive medical/nursing plan is in place for each
individual.

BELLEROSE DAY PROGRAM OPENINGS
TITLE: DAY HABILITATION SUPERVISOR PROGRAM: DAY PROGRAM
SUMMARY OF POSITION: Oversees the day-to-day operations of the group rooms to ensure services are being performed in a timely manner. Supervises all staff assigned to their group
rooms (Coordinator, Specialist, Senior Habilitation & Habilitation Assistants), as well as ensures all documentation complies with person centered planning standards & OPWDD regulations for waiver services. Provides training of staff and completes core competency performance
evaluations in a timely manner. Participates in the life plan meetings.
MINIMUM REQUIREMENTS: BA Degree in a human services field, as well as 2 years of supervisory experience in the field of OPWDD. Successful candidate must be computer savvy, strong communication & interpersonal skills, as well as be able to handle multiple priorities.
